If your job involves wearing an SCBA on a working fire, carrying a stair chair down four flights with a patient, hauling gear up a hill on a wildland incident, or sustaining a long extrication, you have one bottleneck that matters more than almost anything else: oxygen efficiency. How much work you can do before the wall hits.

Beetroot supplementation is one of the few ergogenic aids with a legitimate, well-replicated mechanism behind it. It's not something we sell at RespondWell — our focus is the clinical side of first responder health: TRT, GLP-1s, sexual health, performance medicine. But the research on dietary nitrates is solid enough that we'd be doing you a disservice not to cover it. Think of this as the honest brief on what beetroot actually does, what the data shows, and whether it's worth adding to your protocol.

How It Works

Beetroot is high in dietary nitrate. Once you ingest it, bacteria in your mouth convert that nitrate to nitrite, which gets absorbed into the bloodstream. Under low-oxygen conditions in working muscle — exactly the conditions during a hard call — nitrite converts to nitric oxide (NO).

Nitric oxide does two things that matter here. First, it dilates blood vessels, increasing oxygen and nutrient delivery to working muscle. Second, and more surprisingly, it appears to reduce the oxygen cost of producing the same amount of muscular work — your mitochondria run more efficiently. The net effect: you do the same workload while using less oxygen.

What the Research Actually Shows

The headline finding across dozens of trials: dietary nitrate supplementation reduces the oxygen cost of submaximal exercise by roughly 3–5%. That number has been reproduced consistently since a 2007 study in Acta Physiologica first documented it.

In practical terms, that means any sustained effort in the 60–90% VO2 max range — air-pack work on a fire, prolonged victim drag, repeated stair climbs with gear — costs your body measurably less oxygen. Subjects in time-to-exhaustion studies last 15–25% longer at fixed workloads. The effect is amplified at altitude or in low-oxygen environments, which matters for wildland crews working at elevation.

A 2014 study specifically in firefighter recruits found that beetroot supplementation improved performance on a simulated firefighting circuit and reduced perceived exertion. It's one of the few studies done directly in this population, and it points the same direction as the general literature.

The Blood Pressure Benefit

Hypertension is endemic in fire and law enforcement — shift work, chronic stress, disrupted sleep, and years of adrenaline add up. This is where beetroot has a secondary benefit that's easy to overlook.

A 2013 meta-analysis in the Journal of Nutrition pooled 16 controlled trials and found dietary nitrate supplementation reduced systolic blood pressure by an average of 4.4 mmHg and diastolic by 1.1 mmHg. Those are modest numbers individually. At a population level, a 5 mmHg reduction in systolic BP is associated with roughly a 14% reduction in stroke mortality. For a 45-year-old captain already running 135/85, that's not nothing.

Timing and Dosing

This is where most people get it wrong. Peak plasma nitrite levels occur 2–3 hours after ingestion — this is not a 20-minutes-before-training product. If you take it right before shift, you're mostly wasting it.

Two approaches that work:

Daily dosing is the better fit for first responders. Take it every morning and you maintain a baseline elevation in plasma nitrite throughout the day. Five to seven days of consistent use produces a sustained effect that's actually larger than a single acute dose.

Acute dosing for known events — a training day, a physical assessment, a SWAT op — take it 2–3 hours out.

The research-supported dose range is 400–800mg of dietary nitrate (roughly 6–13 mmol). Concentrated beetroot shots are the most consistent delivery format. Powders work but quality varies significantly — look for products that list nitrate content explicitly on the label, not just "beetroot extract" in milligrams.

Who Gets the Most Out of It

The data favors certain profiles more than others.

Highest benefit: Older responders (40+) with reduced baseline endothelial function, anyone with hypertension or pre-hypertension, firefighters doing sustained SCBA work, and wildland crews working at elevation.

Modest benefit: Younger, well-trained responders with already-high aerobic capacity. The percentage gains are smaller, but still measurable.

Probably skip it: If your demands are almost entirely short, anaerobic bursts — creatine is the better tool for that profile. The two aren't mutually exclusive, but if you're choosing one to focus on, match the supplement to your actual job demands.

One caution worth flagging: if you're on PDE5 inhibitors (Viagra, Cialis, or anything RespondWell prescribes for sexual health), combining with high-dose nitrates can lower blood pressure more than expected. Talk to your provider before stacking.

The Bottom Line

Beetroot isn't in our product line, and we're not pushing it. But the mechanism is real, the trials are consistent, and for first responders doing sustained aerobic work — especially those managing blood pressure — it's a legitimate addition to consider. Daily dosing, a product with verified nitrate content, and give it a week to build.
