If you have been on testosterone for a few months and your provider just flagged a high hematocrit on TRT, you are looking at the single most common lab abnormality in testosterone therapy. It is not a reason to panic, and for most men it is not a reason to stop treatment. It is a reason to measure, adjust, and keep measuring. Clinical studies suggest anywhere from 5% to 20% of men on injectable testosterone will cross the threshold at some point, and the number climbs with higher doses, less frequent injections, untreated sleep apnea, and shift work. For first responders and performance-focused men, the fix is usually a protocol change, not an exit.
What Hematocrit Actually Measures
Hematocrit is the percentage of your blood volume made up of red blood cells. Hemoglobin, its companion marker, measures the oxygen-carrying protein inside those cells. Both show up on a standard CBC, and both tend to rise on testosterone.
Why testosterone raises it
Testosterone stimulates erythropoiesis through two main routes: it increases erythropoietin (EPO) output from the kidney, and it suppresses hepcidin, which frees up more iron for red cell production. The result is a predictable, dose-dependent climb in red cell mass. This is called secondary erythrocytosis — your marrow is doing exactly what it was told to do.
The numbers that matter
Most reference ranges top out around 50–52% for men. Endocrine Society guidance has historically used 54% as the action threshold for men on testosterone therapy — above that, dose reduction or therapeutic phlebotomy is typically recommended. Context matters, though. Baseline hematocrit varies by altitude, hydration, smoking status, and sleep apnea. A firefighter who lives at 6,000 feet, works 24s, and shows up dehydrated for a lab draw can post an inflated number that partly corrects with water and rest.
Is High Hematocrit on TRT Actually Dangerous?
The honest answer is that the risk is real but frequently overstated. The concern is viscosity: thicker blood moves less efficiently and, in theory, raises clot risk. That link is well established in polycythemia vera, a bone marrow disorder. It is less clearly established in testosterone-induced erythrocytosis, where the red cells are structurally normal and the driver is hormonal rather than clonal.
That said, some patients report headaches, facial flushing, ruddy complexion, elevated blood pressure, or a general “heavy” feeling as hematocrit climbs into the mid-50s. Observational data has associated markedly elevated hematocrit with venous thromboembolism, and the TRAVERSE trial — which otherwise reassured on cardiovascular safety, as we covered in TRT and cardiovascular health — did note a higher rate of pulmonary embolism in the testosterone arm. The prudent position: do not ignore it, do not quit over it.
How to Lower Hematocrit Without Quitting TRT
There is a sequence here, and most men never need to get past step three.
1. Split your dose
A single large weekly injection produces a sharp supraphysiologic peak, and that peak appears to drive erythropoiesis harder than the weekly average does. Moving from one weekly injection to two — or to smaller, more frequent subcutaneous doses — flattens the curve. Many patients see hematocrit drift down several points on the same total weekly dose. 2. Reduce the dose
Erythrocytosis is dose-dependent. If your trough total testosterone is running above the top of the reference range, you likely have room to come down without giving up symptom relief. A 10–20% reduction is often enough. This is a conversation with your provider, not a solo adjustment.
3. Address the multipliers
Untreated obstructive sleep apnea, smoking or vaping, chronic dehydration, and high-altitude living all push hematocrit up independently of testosterone. Shift workers stack several of these at once. Treating sleep apnea in particular can meaningfully drop the number — and it improves natural testosterone production at the same time.
4. Donate blood or use therapeutic phlebotomy
A whole blood donation removes roughly 450–500 mL and typically drops hematocrit by about 3 percentage points. Standard donation intervals are every 56 days for whole blood, and most donation centers accept men on prescribed TRT. Two cautions: aggressive repeat phlebotomy depletes ferritin and can leave you iron-deficient and fatigued — the exact symptoms you started TRT to fix — and some evidence suggests frequent phlebotomy stimulates the marrow to rebound harder. Monitoring Cadence: When to Pull Labs
A reasonable schedule for most men on testosterone cypionate: baseline CBC before starting, again at 3 months, again at 6 months, then every 6 to 12 months once stable. If you cross 52%, tighten to every 3 months. If you cross 54%, expect an intervention and a recheck in 6 to 8 weeks.
Draw labs hydrated and at trough — the day of, or the day before, your next injection. A dehydrated post-shift draw can add a point or two of pure artifact and trigger a dose change you did not need.
